TO APPLY FOR STAR'S DAY SERVICES

Begin to gather documents required for admission:
- psychological examination report within three years prior to application date,
- physical examination within one year of application (Download Form)
- residential assessments (if applicable)
- reports from previous service providers
- school records (if applicable)
- reports from ancillary service provider such as speech, occupational, physical therapists (if applicable)
- completed STAR Admission Referral Form (Download Form)
- completed I-9 form required by the Immigration and Naturalization Service to establish identity and employment eligibility (http://uscis.gov/graphics/formsfee/forms/I-9.htm)
- completed release & request of information forms (Download Form)
- guardianship papers (if applicable)
We would be happy to assist with the completion of required forms, and securing needed records. All information is kept confidential. We urge you to share as much information as possible. The more we know, the better we can serve you!

Before applying at STAR we want you to have a thorough understanding of the services we offer. We would be happy to provide references from our participants and their families. We welcome the opportunity to share our successes and challenges with you.
In order to receive services from STAR, the applicant must be third party sponsored or agree to pay the established fee.
- A source for third-party sponsorship is the State of Connecticut, Department of Mental Retardation (DMR). DMR application may be made by calling a toll free number, 1-866-433-8192. Also, the eligibility process can be initiated using the DMR web page www.dmr.state.ct.us.
- Another option for possible third-party sponsorship is the Department of Social Services, Bureau of Rehabilitation Services (BRS). Call toll free at 1-800-537-2549.
- If you have a significant visual impairment, you may contact the Board of Education, Services for the Blind (BESB) at 1-800-842-4510.
- Some applicants and their families have found the process of registering with third party sponsors to be a time consuming and difficult process. Please know that we are available to help in completing required forms, explaining the steps to you, and acting as an advocate if you need additional assistance. If you wish to pay the established fee directly or through a fiscal intermediary, please contact Kari Ryan at (203) 846-9581, ext. 313 for rates.

Once we have received all the required documents and you meet STAR's eligibility requirements, your name will be placed on STAR's internal wait list.
