TO APPLY FOR STAR RESIDENTIAL SERVICES

Begin to gather documents required for admission:

  • psychological examination report within three years prior to application date,
  • physical examination within one year of application (STAR form provided upon request)
  • residential assessments (if applicable)
  • reports from previous service providers
  • school records (if applicable)
  • reports from ancillary service provider such as speech, occupational, physical therapists (if applicable)
  • completed STAR Admission Referral Form (available upon request)
  • completed I-9 form required by the Immigration and Naturalization Service to establish identity and employment eligibility (form provided upon request - note: related documents must be presented)
  • completed release & request of information forms (available upon request)
  • guardianship papers (if applicable)

We would be happy to assist with the completion of required forms, and securing needed records. All information is kept confidential. We urge you to share as much information as possible. The more we know, the better we can serve you!

Develop a list of questions that you have for us!

Before applying at STAR we want you to have a thorough understanding of the services we offer. We would be happy to provide references from our participants and their families. We welcome the opportunity to share our successes and challenges with you.

In order to receive services from STAR, the applicant must be third party sponsored or agree to pay the established fee.

  • A source for third-party sponsorship is the State of Connecticut, Department of Mental Retardation (DMR). DMR application may be made by calling a toll free number, 1-866-433-8192. Also, the eligibility process can be initiated using the DMR web page www.dmr.state.ct.us.

Once we have received all the required documents and you meet STAR's eligibility requirements, your name will be placed on STAR's internal wait list.

Third party sponsors may also have their own separate wait list. Funding, and staff availability and other factors may influence admission. When applying for services, advanced planning is strongly suggested.
